It was done in Photoshop.
You open up the photo, choose the text tool, and put the text in. Then you create a new layer, select the marquis tool and make a square behind the text and use the paint bucket tool to fill it in. Then you take the lasso tool and draw the little triangle things and fill those in as well. Then you merge all the layers, and save it. Pretty simple.
